Instructions to obtain a service agreement for telephone consultations:
1) Click on whichever version of the service agreement you can open with your word processor. This will open the document in your browser.
2) Print out the document to your printer.
3) Fill in the requested information in the bottom half of the agreement. (Please print)
4) Review and sign at the bottom of the page.
5) Either mail or fax the completed agreement back to Dr. Bryce Kaye at 875 Walnut St., Suite 220, Cary, N.C. 27511 (Fax: 919 467 0790).
